A reversionary beneficiary allows you to feel secure in knowing that if you were to pass away—your pension will go straight to the person you nominated.

  • This type of nomination applies to pension accounts only

  • All required sections of the form need to be completed
    You’ll need to complete every required section of the form accurately
  • The person(s) you nominate must be your dependant
  • If your nominated dependant is a child, they are only eligible to receive your superannuation as a series of payments if, at the time of your death they are:
    • Under the age of 18; or
    • 18 or over and they are also:
      • under 25 and financially dependent on you; or
      • permanently disabled (specific criteria apply).
  • In all other cases a child may only receive your superannuation as a single payment upon your death. If this is the case, you cannot nominate them as a reversionary beneficiary
  • No online submissions accepted – we need original signatures

What you’ll need

 

  • Your account number
  • The full name and date of birth of the person you are nominating
  • Access to a printer
  • About 20 minutes of your time to read and complete the form

  1. Download the reversionary beneficiary form

  2. Read the form instructions carefully and complete the relevant form fields

  3. Print, sign and date the form

  4. Post the form to us at:
